A, monthly safety evaluation:
1 Since August, the external engineering projects contracted by the company have started one after another. As of August 3 1 day, although there were no class II obstacles, personal injuries and all kinds of unsafe incidents above, the safety supervision department made random inspections to the project department from time to time, and there were still the following problems: the installation of the maintenance part of the Maliantai wastewater renovation project, the safety officer of the project department lacked the ability to find problems, and the problems existing on site were ignored. Other project department managers are conscientious and responsible, and all work in strict accordance with the safety management requirements for planned maintenance of units. In particular, the daily safety disclosure summary work of Zhongwei Sikouzi and Western Thermal Power Project Department is in place, and the telephone recording of the meeting is transmitted to the company's safety supervision department in time every day. According to the progress of each project, the focus of safety risk prevention is different, and the projects and risk pre-control measures that remind the project department of safety key inspections are improved.

On August 18 -22, the Safety Supervision Department was responsible for the company's special safety inspection working group, and carried out joint self-inspection work together with the Engineering Department, the Safety Department of the Base, the Maintenance Department and the Project Department. 3. On August 3rd, Liupan Mountain #2 unit stopped the high-pressure external cylinder drain pipe orifice flange leakage, and on August 5th, the #2 unit #5 high-conductivity drain pipe to the quick-cooling connection tee was punctured for emergency repair and leakage stoppage, which reflected that our metal supervision work was not in place. The Engineering Department should assist the metal supervisors to improve the device ledger, check the high-pressure welding joints monitored by the unit and the furnace side according to the drawings, and conduct item-by-item investigation to avoid the recurrence of similar incidents caused by missed inspection.
4. The fire of 400VMCC cabinet of Maliantai #2 industrial waste water pump and the burning of stator winding of motor of condensate pump of Liupanshan #2 machine reflect that the overhaul of auxiliary devices of electrical specialty is not carried out seriously, the overhaul depth is not enough, and the daily inspection work is not in place.
5. On August 9, Liupan Mountain # 1 2 units were shut down due to power grid reasons. All staff of Liupan Mountain Base took the overall situation into consideration, carefully investigated the potential safety hazards of the device, and did not expand the accident and breed other device damage incidents, and praised and rewarded them.

2. There are many civilized production of on-site devices, powder accumulation in pipelines and oil leakage points of devices, such as poor sanitation of Liupanshan # 12 furnace devices, and many coal mills, blowers and leakage points. Professionally, the person in charge is urged to carry out rectification according to the division of devices.
3. For long-term outsourcing projects and newly started external projects, the outsourcing team must follow the principle of "who uses and manages", and must sign the Safety Agreement in time and go through the commencement formalities before entering the site for maintenance.
4. From the problems existing in the special safety inspection, it is reflected that the managers of the guarantee system have unclear responsibilities and do not perform their duties in place. The Ansheng Department should conduct regular spot checks and must put forward assessment opinions when problems are found.
