Category I: Mental illness: Alzheimer's disease, mental disorder caused by cerebrovascular disease, mental disorder caused by epilepsy, schizophrenia, mania, depression and bipolar disorder.
Category II: 1, malignant tumor 2, chronic renal insufficiency 3, nephrotic syndrome 4, anti-rejection therapy after organ transplantation 5, chronic leukemia 6, aplastic anemia 7, myelodysplastic syndrome and myeloproliferative diseases 8, systemic lupus erythematosus 9, hemophilia (students and children only)
Category III: 1, chronic active hepatitis, cirrhosis 2, hyperthyroidism or hypothyroidism 3, rheumatoid arthritis 4, hypertension 5, tuberculosis 6, heart disease (rheumatic heart disease, hypercardiopathy, coronary heart disease, cor pulmonale) 7, diabetes 8, Parkinson's disease 9, sequelae of cerebrovascular accident 10, mental illness: anxiety disorder, obsessive-compulsive disorder.
(two) special outpatient procedures
After receiving the Examination and Approval Form for Outpatient Service of Special Diseases from the medical insurance agency → filling in my application column, choose a designated medical institution to fill in the diagnosis, treatment items, inspection items, drug name (including usage), estimated monthly expenses and total expenses for three months according to your own condition, which will be examined and approved by the hospital, signed and stamped → applying for outpatient special diseases for the first time, you must provide the inspection report, diagnosis certificate and "Special Diseases" from the designated medical institutions of Grade II and above within six months.
Outpatient special diseases are audited every 3 months, and outpatient special diseases that meet the first category of diseases are audited every 6 months.
(3) The following information shall be provided for reimbursement:
1. Examination and Approval Form for Special Disease Outpatient Service
2 "application form for the change of outpatient special disease treatment and medication scheme" (the official seal of the designated medical institution shall be affixed);
3. Special bills for outpatient charges made or supervised by financial and tax departments.
4. List of outpatient medical expenses for special diseases, drug prescriptions and inspection reports signed and approved by patients or their families;
5 during the audit period, if the insured is hospitalized, a detailed list of hospitalization expenses shall be provided;
6. Original and photocopy of ID cards of patients and agents;
7. Social insurance card or insurance certificate;
8 medical insurance agencies designated bank savings account.
