China Bank's application is divided into two parts: bank filling and customer filling. The following is the part filled in by the customer:

Name of the applicant/agent; Contact telephone number;

The address of the applicant;

Type and number of certificate (ID card, passport-);

The currency and amount of the application for foreign exchange purchase; (USD 5000.00)

Foreign exchange payment method and RMB payment method;

Countries/regions to be visited;

Purpose of purchasing foreign exchange (see the last page of the application form);

Signature of the applicant/agent; Dating.
