Legal basis: Notice of the Ministry of Civil Affairs on the Identification and Treatment of Veterans Returning to their hometowns with Illness Article 2 Conscripts and junior noncommissioned officers who have difficulties in returning to their hometowns with chronic diseases may apply to the civil affairs department of the people's government at the county level for the treatment of veterans returning to their hometowns with illness, and submit or possess the following materials:
(1) I apply;
(2) household registration book;
(3) veteran certificate;
(four) the military hospital certificate, especially one of the following:
1. The registration form of veterans with chronic diseases recorded in veterans' files or the certificate issued by the military system hospital that they suffered from chronic diseases during their service (to be confirmed by the hospital or the health department at a higher level);
2 recently copied from the military system hospital and stamped with the seal of the medical record management department.
(5) A copy of recent chronic diseases (especially those recorded in the original military hospital certificate) stamped by the hospital medical record management department, relevant physical examination reports, diagnosis conclusions, etc.
