(1) The business license of an enterprise as a legal person, the business license of an unincorporated enterprise, the registration certificate of a public institution, and the documents submitted by other organizations to prove their legality are all photocopies, and the originals shall be presented;

(2) A copy of the organization code certificate and show the original;

(3) A copy of the ID card of the agent and show the original;

(4) An application form for reporting the loss of a loan card signed by the legal representative or person in charge and stamped with the official seal;

Note: If you did not attend the annual review or have any information changes, please refer to the materials that should be submitted for the annual review of the loan card to prepare and submit them together.

All the above copies shall be copied on A4 paper and stamped with the official seal.
