Persons who meet the conditions for assistance must submit a written application to the village (neighborhood) committee where the household registration is located at the end of each quarter after the relief object has been reimbursed and compensated by various medical treatments such as rural cooperative medical care;

2, and provide the household registration book or identity card, five guarantees certificate, low guarantee copy, rural cooperative medical reimbursement and all kinds of compensation vouchers, people's hospitals, traditional Chinese medicine hospitals, township hospitals, disease diagnosis, hospitalization invoices, hospitalization summary and other relevant documents, social assistance and other supporting materials, as well as other supporting materials that the county civil affairs department deems necessary;

3, the village (neighborhood) committee or township social assistance should be after receiving the application and necessary supporting materials for investigation and preliminary examination, and guide them to truthfully fill out the "medical assistance approval form", signed opinions, and publicity for 3 days;

4. The Township People's Government shall carefully examine the materials declared by the neighborhood committees and the Application and Approval Form for Rural Medical Assistance, and re-investigate those who are in doubt. If they meet the requirements, they shall return to the village committee for publicity for 3 days, and report to the county civil affairs bureau after signing their opinions. If they do not meet the requirements, they shall return the materials and explain the reasons on the application and approval form.

5, the county civil affairs bureau according to the application for medical assistance, relief object category, personal medical expenses, etc., organize subsistence allowances and financial shares to conduct a comprehensive review of the submitted materials, to meet the conditions of relief, entrust the village neighborhood committee to publicize in the bulletin board for 3 days, and approve the implementation of relief after no objection, and to return the materials that do not meet the conditions, and explain the reasons;

6, adhere to the convenience of the masses, the principle of enthusiastic service, immediately after the examination and approval of medical assistance funds directly into personal accounts.

Those who are qualified to apply for serious illness relief from the Women's Federation may prepare the following materials and submit the application:

1, aid application form in triplicate;

2. A copy of the applicant's ID card or household registration book; If the application is made by an entrusted agent, a copy of the ID card of the applicant and the entrusted agent shall be provided at the same time;

3. Copies of medical records and medical certificates (including diagnosis and treatment) issued by hospitals above level 2 and original invoices issued by hospitals above level 2;

4. A copy of the sick woman's urban and rural minimum living guarantee certificate or the original family poverty certificate issued by her village.

Interim Measures for Social Assistance

Article 28 The following persons may apply for relevant medical assistance:

(a) the minimum living security family members;

(2) destitute dependents;

(3) Other persons with special difficulties as stipulated by the people's governments at or above the county level.

Twenty-ninth medical assistance shall take the following forms:

(a) to subsidize the individual contributions of the relief object to participate in the basic medical insurance for urban residents or the new rural cooperative medical system;

(two) to provide subsidies for the basic medical expenses that are difficult for individuals and their families to bear after the assistance object has been paid by the basic medical insurance, serious illness insurance and other supplementary medical insurance.

The standards of medical assistance shall be determined and promulgated by the people's governments at or above the county level in accordance with the level of economic and social development and the situation of medical assistance funds.
