1. Original and photocopy of industrial and commercial business license or other approved practice certificate

2. Original notice of business commencement approved by the administrative department for industry and commerce

3. Original and photocopy of resident ID card, passport or other legal documents for the head of the branch

4. Copy of industrial and commercial business license and tax registration certificate of the head office

5. Production and business address and house. Original and photocopy of the site certificate (house property certificate, land certificate or lease agreement)

6. The local branch in other places shall also provide the original and photocopy of the unified code certificate of the organization
