If the cardholder terminates the social insurance relationship in our province for various reasons, he needs to go through the cancellation procedures of the social security card. The cardholder must hold the social security card, the original valid identity document and the certificate of termination of the social insurance relationship, first go through the cancellation procedures of the bank account at the network outlet window of China Bank, and then go through the cancellation procedures of the social security card at the comprehensive service window of the social security card, and return the social security card. In case of entrusting others to act as agents, the original valid identity certificate of the agent shall also be provided. The specific procedures are as follows:

(1) The handler or agent shall go to the social security card comprehensive service window to cancel the social security card with the social security card, the original ID card, the copy of the ID card and the certificate of termination of the social insurance relationship.

(2) The handler or agent shall go to the social security card business window of the cooperative bank to cancel the bank account with the social security card, the original valid ID card, the copy of the ID card and the certificate of termination of the social insurance relationship.

(3) The handler or agent shall go to the social security card comprehensive service window to handle the cancellation procedures of the social security card, and fill in the Shanxi Social Security Card Business Processing Form when handling.

(4) After the staff of the social security card comprehensive service window confirms that the information accepts the cancellation application, they will go through the cancellation procedures and take back the social security card. If you entrust others to do it, you should also provide the original valid identity certificate of the agent.
