Procedure for reissue of Certified Tax Agent Qualification Certificate

1. Application procedures

(1) Download the application form for re-issuance or replacement of certificates

(2) Fill in the form and apply to the examination center with relevant certificates

2. Relevant materials must be prepared for re-issuing and replacing certificates

(1) Application form for re-issuing and replacing certificates (see attachment)

(2) Original valid identity document and Copy

(3) Original and copy of admission ticket

(4) Original and copy of declaration of loss of registration (replacement certificate).

3. Loss declaration mode

Name ×× passed the ×× annual ×× examination, certificate number ××××, the certificate has been lost, and the declaration is invalid.
