Take two one-inch color photos with ID card and personal bareheaded, pay the medical examination fee at the toll booth, get the medical examination form, paste the photos (the other one is for health certificate), and fill in the basic information such as name and gender.

If the physical examination results meet the conditions for issuing health certificates, health certificates will be issued after passing the training of health laws and regulations organized by health supervision institutions. If the physical examination results do not meet the conditions for issuing a health certificate, a health certificate shall not be issued.
