When a legal person applies for bankruptcy, it shall file a bankruptcy application with the people's court, and submit the bankruptcy application and relevant evidence.

The bankruptcy application shall specify the following items:

Basic information of the applicant and the respondent;

(4) Other matters that the people's court considers should be specified.
