Handling materials include:
1. Original valid ID card and its front and back copies;
2, the township (town) people's government or street offices issued by the "Shanghai application for maternity insurance and family planning treatment audit form" or "Shanghai application for maternity insurance and family planning treatment certificate";
3. If you give birth in this city, you need to carry the "Reproductive Medicine Certificate" issued by a medical institution; In other provinces and cities, it is necessary to bring the original and photocopy of the discharge summary or medical record, and the "Birth Medical Certificate" issued by a qualified medical institution recognized by the health administrative department. Those who give birth abroad or in Hongkong, Macao and Taiwan Province Province need to bring medical records and other materials and Chinese translations issued by local medical institutions indicating the birth situation (dystocia, natural delivery or abortion);
4. Copy (discount) of the real-name registration system settlement account card opened by me in the designated financial institutions in this city (ICBC, China Construction Bank, Agricultural Bank, Postal Savings Bank, Shanghai Bank, Shanghai Rural Commercial Bank, Bank of China, China Merchants Bank, Minsheng Bank, Bank of Communications, China Everbright Bank and Huaxia Bank);
5. If you entrust others to handle it, you need to bring the power of attorney and the original and copy of the client's valid ID card.
