1. The customs declaration agent shall provide: the power of attorney for customs declaration in triplicate (stamped with the official seal), a blank customs declaration form (stamped with the special seal for customs declaration), an invoice and a packing list. If the goods have passed the statutory inspection, a customs clearance form or a certificate of replacement shall also be provided.

The power of attorney for customs declaration and the blank customs declaration form can be bought at the customs or sent to you by the customs broker. You don't need to fill it out, just stamp it.

2. Materials required for write-off: memo, customs declaration form, write-off form and write-off information form. This is the most basic information, in addition to this information, all localities may need other information. You'd better ask SAFE.
