Did the forwarder lose the verification form and customs declaration form? We have just encountered a similar situation.
You handle this matter like this:
1.) First find the docking personnel of your foreign exchange bureau who are responsible for the verification of your enterprise, tell them this situation and apply for an extension;
They will tell you the corresponding procedure. We take the original customs declaration form (in duplicate), and they can issue a certificate that the verification form is lost. There is no charge, but formalities and documents are needed.
2) Contact your freight forwarder, tell them the consequences of not writing off, and ask them to help fill in the customs declaration form;
This requires a fee, the exact amount is unknown; I didn't know how much ours cost, so I scared the freight forwarder into asking for it.
Lost a lot of money, some are not very professional;
3.) You didn't go directly to the tax bureau for tax refund, did you? You need to go to the local directly under the State Taxation Bureau to issue a tax refund certificate and fill in the customs declaration form.
Enterprise tax refund is needed.
4.) Requirements for documents:
Materials to be submitted for the enterprise's non-tax refund certificate: A. Copy of customs declaration form (looking for freight forwarder)
B. bill of lading;
C. commercial invoice;
D. customs clearance data sheet in the electronic port card;
E. enterprise tax refund form;
F. information description;
G. packing list.

It's really troublesome to lose the verification form. Don't just try to save money. Find a good forwarder instead of the cheapest one.
I ran back and forth four times to get the certificate. My colleague also knows people from the tax bureau and has to deal with government departments.
Be mentally prepared. Pay attention to refunds. We were unlucky, and the freight forwarder who lost the ticket B mistyped the unit price.
