(1) Write-off form for import payment of foreign exchange, which shall be filled in as required and stamped with business seal by the paying bank;

(2) Original import contract and invoice;

(3) Confirmation agreements such as contract cancellation, modification and liquidation signed by the buyer and the seller;

(4) Foreign-related income declaration form stamped by the bank;

(5) A foreign exchange settlement memo or collection notice stamped by the bank and marked with the words "import tax refund";
