For the convenience of reading, the cerebrospinal fluid tryptophan test in the following article has been automatically replaced by TrpT. You can click here to restore the original appearance, or you can use the remarks 1 to summarize that the mixture of cerebrospinal fluid, concentrated hydrochloric acid and formaldehyde is positive when it comes into contact with sodium nitrite solution. If there is a brownish yellow ring or no colored ring at all, it is negative. The positive rate of this test in tuberculous meningitis can reach about 90%, but it is not specific. In Japanese encephalitis cases, the positive rate of tryptophan test is only 15%. Nevertheless, this test can be used as a powerful reference for the diagnosis of tuberculous meningitis. It should be noted that the test can also show a positive reaction when the cerebrospinal fluid contains pus, blood or yellowing.

5. Test cerebrospinal fluid.

Principle 6 TrpT tryptophan is condensed with formaldehyde, oxidized by ferric chloride, and forms norHarman with fluorescence, and its fluorescence intensity is measured by fluorescence spectrophotometer.

7 Reagent (1)0.6mol/L trichloroacetic acid (TCA): weigh 98g trichloroacetic acid (AR) and add a little distilled water to dissolve it, then add distilled water to 1000ml and mix well. If there is turbidity, filter with filter paper. Store at 4℃.

(2)0.67mol/L formaldehyde: absorb 5.6 ml formaldehyde solution (36%, Ar), add distilled water to 100ml, mix well, and store at 4℃ for 3 months.

(3)6mmol/L ferric chloride: Weigh 162 mg ferric chloride (FeCl3 6H2O, Ar) and add 0.6 mol/L. After TCA is slightly dissolved, add it to 100ml and store it at 4℃ for 3 months.

(4) Ammonia (25% ~ 28%, argon).

(5) 1.0 mmol/L tryptophan standard storage solution: accurately weigh 20.4mg L tryptophan (tryptophan, MW204.23, chromatographic purity, Shanghai Biochemical Reagent Store), add about 10ml distilled water, and add 0.76mL ammonia water. When tryptophan is completely dissolved, pour it into a 100ml volumetric flask and add distilled water. Store at 4℃ for 3 months.

(6) 10μmol/L tryptophan standard application solution: absorb 1.0ml tryptophan standard storage solution, put it in a 100ml volumetric flask, add distilled water to the scale, mix well, and prepare it on the same day.

(7) 10g/L tryptophan injection (Shanghai changzheng pharmaceutical factory).

(8) Medicinal glycerin and medicinal vaseline.

8 Operation method: Tryptophan test operation method was adopted.

9 The normal value is negative.

The clinical significance of 10 test results is positive: tuberculous encephalitis (clear and colorless), cerebrospinal fluid containing pus or blood.

1 1 Note that after lumbar puncture of CSF, add 2ml and 5mg/ml sodium sulfosalicylate to shake the precipitated protein evenly, immediately put it in curling, and put it in the refrigerator at -20℃ within 10h or store it at -40℃ ~-70℃ as soon as possible. No more than 2 months. Blood pollution should be avoided (because the content of Asp in red blood cells is 200 times higher than that in plasma). Ask to stop taking drugs and take samples after one week.

More than 20 kinds of amino acids can be separated and determined by HPLC and amino acid analyzer. The normal contents of some reports are quite different, which may be caused by different instruments and methods, as well as different treatment and preservation conditions of samples, and the changes of amino acid concentration in cerebrospinal fluid of some diseases are still contradictory, so the determination method and clinical significance of amino acids in cerebrospinal fluid need further study.

The content of amino acids in cerebrospinal fluid increases with age, including glutamic acid, serine, histidine, threonine, arginine and cystine, but there is no gender difference.
